I confirm that the above information I have submitted to be correct and truthful. Discs will be tested on several machines and if no fault is found, will be returned and additional postage charged (Some low-end machines cannot cope with newer technoligies being included on some of the latest releases). Failure to fill in all the information on this form will also result in the disc being returned to sender at senders expense.
